Utah Live Bands pays independent contractors with a direct deposit into your savings or checking account.  To be paid, please fill out the form below and payments will show up in your account right after a gig like magic!

In addition, if we are required to obtain a W-9 form from you, please fill one out, sign it and scan and email it to us.  The W-9 form can be filled out here http://www.irs.gov/pub/irs-pdf/fw9.pdf.  Once filled out, you will need to print, sign and email it back.
